Alcohol and recreational drugs increase drowsiness and risk.

Certain people need extra caution: older adults (more prone to falls, confusion, and low blood pressure), people with heart disease or a history of arrhythmia, those with glaucoma or enlarged prostate (anticholinergic effects), and anyone with a seizure disorder. If you have liver or kidney problems, dose adjustments may be needed. Your doctor may order an ECG if you have cardiac risk factors or are on other drugs that affect the heart.

Pregnancy and breastfeeding require a careful risk–benefit talk with your provider. Some TCAs have been used in pregnancy, but decisions should be individualized.

Practical tips for safer use

Start low and go slow—many side effects ease after the first few weeks. Take trimipramine at night if it makes you sleepy. Don’t stop suddenly; withdrawal can cause anxiety, sleep problems, and flu‑like symptoms. Carry a list of your medicines and show it to every clinician you see so they can check for interactions.

Watch how you feel when you start or change dose: note dizziness, heart changes, severe mood swings, or thoughts of self‑harm, and tell your prescriber right away. Avoid heavy alcohol use and be cautious driving until you know how it affects you.
